Fill out these forms with the correct information AFTER you have signed up your new phone. Only proceed if you are at STEP 4 in our Instruction Package and wish to change your cell plan to the discounted one.
Make certain you have created an account with the carriers online portal for customers. Ensure the account is working and you can view details before proceeding.
Please ensure you have tested your new sim card and/or phone and that it is working and activated on the carriers network. Inspect the phone for any defects or problems which may interfere with the plan change.
